How Traditional Ultrasonography Works

To understand how ultrasound works, consider the manner in which bats navigate. They have poor vision. When they fly, they emit high-pitched sound waves that bounce off other objects. These sound waves help bats to construct an image of their surroundings.

In a similar fashion, medical ultrasonography uses high-frequency sound waves that bounce off the structures in your body. These structures include your organs, muscles, tissues, and blood vessels. The ultrasound waves are produced by a device called a transducer. This device receives the returning signals and transmits them to a computer. The computer converts the signals into 2-dimensional images called B-mode images.

B-mode images display your body’s internal structures and thus, help doctors identify abnormalities and diagnose a variety of medical conditions. But they are limited in the context of identifying Yaz and blood clots because they do not show moving objects. For this reason, Doppler ultrasonography is often used. Doppler Ultrasonography For Diagnosing Yaz Blood Clots

While conventional sonography bounces sound waves off internal structures, Doppler ultrasound bounces them off moving objects. The ultrasound waves penetrate your tissues and bounce off red blood cells before returning to the transducer. The signals are then transmitted to the computer, which generates 2-dimensional pictures showing the direction of blood flow and its velocity.

These images help doctors identify areas where the passageway through your veins or arteries has narrowed. Duplex Ultrasound: Combining The Two Approaches

Duplex ultrasound is performed using a Doppler transducer. However, rather than merely producing images of blood flow, this test also produces images of your internal structures. Essentially, it combines aspects of conventional ultrasound and Doppler ultrasonography.

This combined approach provides physicians with a clearer image of your venous or arterial network near affected structures (i.e. lungs, brain, heart, etc.). This is the reason radiologists commonly rely on this test to study vascular function.

All forms of sonography-based imaging are noninvasive and harmless. That said, other tests are often necessary in order to form a conclusive diagnosis of blood clots. For example, ultrasound is useful for identifying thrombi in the lung arteries. But it is usually accompanied by more invasive tests, such as a ventilation-perfusion (V/Q) scan and pulmonary angiogram.

Inflammation occurs when stones prevent bile fluid from exiting the gall bladder. This triggers sharp abdominal pain, which may be accompanied by nausea and vomiting. Doctors typically recommend gallbladder removal surgery. However, while removing the organ will address the inflammation, it will not resolve stones that migrate into the common bile duct. To remove these stones, endoscopic retrograde cholangiopancreatography (ERCP) and endoscopic sphincterotomy (ES) are necessary.

Gallstones In The Bile Duct

Most stones form within the gallbladder and remain inside the organ. They can, however, migrate into the bile duct. This can lead to several problems if the stones form a blockage in the duct. First, an obstruction that prevents bile from passing into the small intestine will cause bile fluid to back up. This can trigger further inflammation of the gallbladder as well as liver damage.

Second, a blockage can cause infection within the bile duct. This is called cholangitis. Bacteria from the duodenum (the upper segment of your small intestine) enters the duct and climbs upward through the biliary tract. During times when bile flows unobstructed, this bacteria is held in check. A blockage, however, allows it to fester and turn into an infection.

Third, stones that migrate further along the bile duct can become lodged near the sphincter of Oddi, the opening that spills into the duodenum. When this happens, digestive enzymes secreted by the pancreas are prevented from passing. Trapped within the pancreatic duct, the enzymes become activated and begin to digest the cells of the pancreas. This is known as pancreatitis, a dangerous condition that requires immediate medical care.

Note that the circumstances described above cannot be resolved by simply removing the gallbladder. ERCP and ES are necessary to retrieve the offending stones from within the bile duct.

How ERCP and ES Are Done

Unlike gallbladder removal surgery, ERCP and ES are done without making incisions. Instead, a long, flexible instrument called an endoscope is placed in the mouth and directed down the esophagus. This instrument is equipped with a tiny camera on its end that sends images back to the doctor. The doctor will use these images to help him or her direct the endoscope through the stomach and into the small intestine until it reaches the sphincter of Oddi.

Once the endoscope arrives at the bottom of the bile duct, the doctor will pass a catheter through it. Contrast dye is introduced into the duct through the catheter. This dye helps to illuminate the bile duct, and displays gallstones on x-rays. Another instrument is then sent through the catheter to retrieve the stones.

The opening of the bile duct near the duodenum is often too narrow to thread the retrieval instrument. In such cases, the doctor will pass another instrument through the catheter and make an incision in the opening. Doing so widens it and allows the doctor to direct the retrieval tool into the bile duct to collect the stones.

This last step (endoscopic sphincterotomy) can also be accomplished with balloon dilation. Rather than making an incision near the opening of the duct, a balloon is inflated to widen the passage. This method is preferable because the opening can be widened without cutting through muscle. Some hospitals, however, do not offer it.

Yaz gallbladder problems can be resolved by removing the organ (though doing so is not without consequences). Bile duct stones, on the other hand, require ERCP with endoscopic sphincterotomy. Bleeding, infection, and even pancreatitis can result.
